As we age, our lifestyle choices play a bigger role in our long-term health, especially if you have risk factors like family history or existing conditions. Around age 40, your risk of heart disease increases, making it the perfect time to take proactive steps.

This service is designed to empower you to take charge of your heart health with expert guidance and a personalised care plan.
A ‘heart-healthy’ lifestyle includes:
- Regular physical activity
- Balanced diet rich in fruits, vegetable, and whole grains
- Stress management
- Maintaining a healthy weight
- Not smoking
How to get started
Request the service to complete a heart health risk assessment. If eligible, a Teladoc Health Cardiac Nurse will guide you through a personalised care plan. You’ll also need to visit your GP for a heart health check to record key stats like blood pressure, cholesterol and BMI (at your own cost).

How does it work?
Request the Heart Coach service and we’ll book you an appointment to get this underway.

Initial assessment
Open a case online or by phone, complete a risk assessment, and if eligible, an intake will be scheduled with one of our Cardiac Nurses.

Holistic approach
Your nurse will guide you through lifestyle changes and ask you to complete a heart health check with your GP (at your own cost). If appropriate, the nurse will also recommend other medical services we provide through Connected Care.

Follow up discussion
Together, you’ll create a personalised care plan and get 12 months access to a heart health support app, technology manufactured by Cardihab.

Regular check-ins
Meet virtually with your Cardiac Nurse each month for four months to track your progress.

Follow-up review
Your final monthly check in with your nurse will include a review of your Heart Coach journey and a summary report will be provided.
